When the context window approaches 80% utilization, tool-call count nears 80, or recall degrades (re-reading files you already processed), stop starting new work. Finish the current atomic action, then write HANDOFF.md at the working-directory root. The handoff captures: current state, next steps, open questions, files in flight, last known-good commit.
